Confirm the reported true-peak clipping condition and identify every affected asset or task.

Measure oversampled true peak after every lossy encode and sample-rate conversion.

Pause or quarantine affected outputs so the problem cannot propagate to another production stage.

Lower or limit the offending peaks, encode again and repeat the measurement.
05
Record the owner, evidence, decision, version and completion time in the production log.
