True-peak clipping
Failure signal
A true-peak clipping condition is reported or detected during audio.
Likely cause
The controlling record, technical state or approved specification for audio does not match the current asset or workflow.
Verification
Measure oversampled true peak after every lossy encode and sample-rate conversion.
Corrective action
Lower or limit the offending peaks, encode again and repeat the measurement.
Prevention
Add this verification to the audio gate, retain immutable evidence and assign one accountable owner.
